The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.    Arctic Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 315357723, citation number 1, item number 2 and was affirmed as a final order on February 7, 2012, with respect to a workplace located at 9731 NW 114th Way, Miami, FL 33178.
