29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) All American Gutter Company Inc. at 209-211 Rock Bridge Lane, Windsor, CO 80550: On or about December 20, 2013, All American Gutter Company, Inc., did not ensure that its employees were protected from falls while working at heights greater than 6 feet. Employees of All American Gutter Company, Inc. were exposed to fall hazards in excess of 10 feet while engaged in gutter installation activities on the roof of a residential building with no fall protection in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: (a) All American Gutter Company Inc. at 209-211 Rock Bridge Lane, Windsor, CO 80550: On or about December 20, 2013, All American Gutter Company, Inc., did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. Workers were observed using a ladder to access the roof of a residential building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: (a) All American Gutter Company Inc. at 209-211 Rock Bridge Lane, Windsor, CO 80550: On or about December 20, 2013, All American Gutter Company, Inc., did not ensure that its employees were using ladders properly. Two employees were observed using the top step of a step ladder to access the roof of a residential project, while conducting gutter installation activities. Employees were exposed to fall hazards in excess of 8 feet.
