29 CFR 1910.119(f)(1)(i)(B): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address normal operations:    (a) Tank Farm: The employer did not develop and implement an adequate written procedure that provided clear instructions for safely conducting tanker truck loading operations for the ethanol loading process, such as but not limited to, procedures for selecting tankers for loading, verifying adequate tanker capacity, making line connections and equipment operation, on-going monitoring of the loading process, and emergency shut-down procedures, on or about 12-19-13.
