29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a.  Located on the West end of Prairie Norton Elementary School in Galloway, Ohio, the general contractor failed to ensure that the employees working on the site were protected from falling into a 30-1/2 inch wide geothermal well, thereby exposing the employees to a trip and/or fall hazard of nine feet.
