On or about 12/20/2014 employees at the Sullivan Place Centers construction project were exposed to eye punctures.  Employees were operating pnuematic air nailers without eye protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about 12/20/2013 employees at the Sullivan Place Centers construction project were exposed to approximately 12' falls.  Employees were engaged in roofing activities without fall protection.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.  On or about 12/20/2013 employees at the Sullivan Place Centers were exposed to approximately 12' falls.  Employee's ladder footing exceeded the one-quarter length requirement.
