29 CFR 1910.22(a)(2): The floor of every workroom was not being maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable:     In the kitchen, serving line, on or about December 30, 2013 and at times prior thereto, employees serving and preparing food were exposed to a slip hazard, while standing on the wet floor without proper floor mats.

29 CFR 1910.212(a)(1): Guards on machines were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:     In the kitchen, North side, on or about December 30, 2013 and at times prior thereto, employees engaged in mixing dough were exposed to an amputation hazard, while using a Hobart H600T mixer without a guard for the rotating auger.

1910.305(g)(1)(iv)(A) A flexible cord was being used as a substitute for the fixed wiring of a structure:     In the food pickup area, West side, on or about December 30, 2013 and at times prior thereto, employees retrieving condiments were exposed to an electrical hazard, while opening a refrigerator that was plugged directly into an extension cord.
