29 CFR 1910.1047(d)(2)(i): Initial monitoring was not performed to determine accurately the airborne concentrations of ethylene oxide to which employees may be exposed:  a) Surgical Prep Area - Employees are exposed to ethylene oxide when operating an Andersen AN74i Anprolene gas sterilization unit and were not provided with initial exposure determination, on or about 12/27/13.  b) Basement - Employees are exposed to ethylene oxide when loading and unloading surgical equipment into a 32 gallon garbage pail for gas sterilization with ethylene oxide and were not provided with initial exposure determination, on or about 12/27/13.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1047(h)(1)(i): A written plan for emergency situations was not developed for each ethylene oxide workplace where there was the possibility of an emergency:  a) Surgical Prep Area - employees exposed to ethylene oxide when loading and unloading surgical equipment into an Andersen AN74i Anprolene gas sterilization unit were not provided with a written emergency plan specific to ethylene oxide; on or about 12/30/13.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Facility - Employees are exposed to various chemicals such as but not limited to ethylene oxide without being trained at the time of their initial assignment; on or about 12/30/13.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
