29 CFR 1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:         a)  On or about December 30, 2013, at the above located workplace, employees worked with and around a 120 volt floor mat washer. The floor mat washer had an open junction box cover.  The condition on the junction box was not in accordance with instructions included in the listing or labeling. Employees that worked at or around the floor mat washer were thereby exposed to an electrical hazard.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosure.  In wet locations the enclosure was not weatherproof.        a)  On or about December 30, 2013, at the above located workplace, employees worked with and around a 120 volt floor mat washer. The floor mat washer was used in a wet location and had an open junction box cover. Employees that worked at or around the floor mat washer were thereby exposed to an electrical hazard.

29 CFR 1910.1200(g)(5): The chemical manufacturer, importer or employer preparing the material safety data sheet did not ensure that the information recorded accurately reflects the scientific evidence used in making the hazard determination.      a) On or about December 30, 2013 at the above located workplace employees were required to work with and exposed to 1) Delta Rust Inhibitor (Delta Sonic Control  #507), Manufactured by Delta Sonic Inc., and 2) Sonic Drying Agent (Delta Sonic Control  #404), Manufactured by Delta Sonic Inc. The manufacturer's/employer's Material Safety Data sheets for these dissimilar hazardous chemicals were identical with the exception of the name/identity of the chemical. The employees were thereby exposed to at least one chemical with unknown ingredients and hazards.
