29 CFR 1926.404(b)(1)(ii): Receptacles on a three-wire, dual voltage portable or vehicle-mounted generator rated at 5kW or more, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, were not be protected with ground-fault circuit interrupters. (a) Mosa Generator Serial Number 0000021255 - Employees used the three phase generator with no GFCI protection to provide power to the Plano Matic P703 paving machine. Employees operated the paving pachine to smooth out the polyurethane binder and rubberized granules for the field hockey pitch and were exposed to the hazards of electrical shock on or about December 27, 2013. (b) Olympian Generator Serial Number oly00000EGEL00369 - Employees used the three phase generator with no GFCI protection to provide power to the SMG M 6004S Mix Matic mixer. Employees operated the mixer to combine the polyurethane binder and rubberized granules for the field hockey pitch and were exposed to the hazards of electrical shock on or about December 27, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.403(g): Electrical equipment which was used did not have other markings giving voltage, current, wattage, or other ratings as necessary: (a) Olympian Generator - The employer used the generator to provide power to the SMG M6004S Mix Matic. The employer did not ensure that durable markings giving the voltage, current, wattage or other ratings were affixed to the generator on or about December 27, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
