29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) On or about December 30, 2013, in the workplace:  Lockout was performed on the Two Cull Long Belt (Disconnect S-14), and the employer had not established a program of energy control procedures, employee training, and periodic inspections.  Abatement certification is required for this item.

29 CFR 1910.303(g)(1)(i)(A): The depth of the working space in the direction of access to live parts was less than indicated in Table S-1. Distances were be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed.  a) On or about December 30, 2013, in the workplace:  Access to disconnect Switches for the Large Incline (D-2), South Top Conveyor (D-9), and Hydro Pump (D-10) was prevented by storage of garbage and a pallet jack.  The material was stored directly adjacent to the disconnect switches.  Abatement certification is required for this item.
