On or about December 23, 2013, at Outback Steakhouse Restaurant located at 180 Pakis St, Hot Springs, AR 71913, the employer did not ensure an employee working on awning installation was properly protected by the use of a hardhat. Employee was exposed to struck by hazards.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. On or about December 23, 2013, at Outback Steakhouse Restaurant located at 180 Pakis St, Hot Springs, AR 71913, an employee working on awning installation was using an electric power hand held drill with damaged extension cord. The employer did not ensure an employee was operating an electric power hand held drill free of hazards. Employee was exposed to electrical hazards.
