29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:    On December 29, 2013 at the above jobsite, while installing sections of rebar, the employer did not assure that each employee was protected from falling by requiring the use of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about December 29, 2013, at the above jobsite, an employee was exposed to a fall hazard while allowed by the employer to conduct rebar reinforcement work at a height of approximately 14 feet 6 inches without receiving training on the recognition of fall hazards prior to commencement of the work.
