29 CFR 1917.23(d)(4): Appropriate warning signs or physical barriers were not posted at all means of access to prevent inadvertent employee entry into spaces that had been identified as having hazardous, flammable, or oxygen deficient atmospheres. (a) On or about January 3, 2014 the inspection revealed that the employer did not ensure that all openings into tanks were marked with appropriate warning signs or that access was prevented by physical barriers. Located at several tank access points at 11040 N Lombard in Portland, Oregon.
