29 CFR 1910.23(a)(8)(ii): Floor holes into which persons can accidentally walk were not guarded by a floor hole cover of standard strength and construction: (a) IFCO Systems NA dba Pallet Companies Inc, at 8221 East 96th Avenue Unit C, Henderson, CO: On and preceding 1/6/14, IFCO Systems NA dba Pallet Companies Inc did not ensure that floor holes into which persons can accidentally walk were guarded by a floor hole cover of standard strength and construction. Several four inch deep floor troughs ran across the maintenance shop floor. Grates covered portions of the floor troughs. Powered industrial trucks had damaged sections of the floor trough grates, resulting in ten inch long by six inch and ten inch long by ten inch wide floor openings. The employer did not ensure that the floor holes were guarded by intact floor hole covers. This condition exposed employees to floor hole hazards.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) IFCO Systems NA dba Pallet Companies Inc, at 8221 East 96th Avenue Unit C, Henderson, CO: On and preceding 1/6/14, IFCO Systems NA dba Pallet Companies Inc did not ensure that respirators with tight-fitting facepieces were not worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function. The employer allowed employees to voluntarily wear respiratory protection when exposed to wood dust. One employee wore a Moldex N95 filtering facepiece respirator when operating the Smart Products notcher. The employee had facial hair that interfered with the seal of the respirator facepiece. The employer did not ensure that the employee did not have facial hair that interfered with the seal of the respirator facepiece. This condition potentially exposed the employee to a respiratory hazard.
