29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: (Construction Reference 1926.103)  (a) Throughout the facility; The employer did not provide for employees to be medically evaluated prior to using tight fitting elastomeric half-face pieces possibly leading to cause or compromise injury or illness such as, but not limited to cardiac arrhythmias, asthma, chronic bronchitis, neurological disorders, and psychological disorders.

29 CFR 1910.134(i)(5)(iv): The employer did not ensure that the person authorized to perform the necessary sorbent bed and filter changes filled out a tag indicating by the signature and date that maintenance changes were performed:  (a) Bullard Free-Air Pump for Sandblasting Booth; The employer did not ensure breathing air quality by maintaining a tag at the compressor identifying the date filter maintenance was completed and the signature of the authorized person performing the maintenance.  This may lead to poor performance of the supplied air pump and/or compromise in supplied air.
