(a) Location - Warehouse: On or about January 6, 2014, employees had to access products that were stored on a catwalk. The catwalk was 89 inches high (7'5") and did not have a guard rail.

29 CFR 1910.28(a)(1): Scaffolds shall be furnished and erected in accordance with this standard for persons engaged in work that cannot be done safely from the ground or from solid construction, except that ladders used for such work shall conform to 1910.25 and 1910.26. (a) Location - Warehouse: On or about January 6, 2014, employees had to climb onto pallets stacked with grain bags to get the bags for customers. The level that the employees had to stand on was 58 inches high (4'10") and they were not protected from falls. (b) Location - Warehouse: On or about January 6, 2014, employees had to climb onto stacks of hay bales to get the bales for customers. The level that the employees had to stand on was more than 10 feet and they were not protected from falls.

Location - Warehouse: On or about January 2, 2014, employees were operating a Datsun forklift and had not been properly trained on how to use it in that either formal instruction or practical exercises were not provided.
