29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by in-running nip point(s) of rotating rollers:   a/ Multi-Plastics/Avenel NJ Site - Production PS1 Extruder Area:   1/  Rotating rollers approx. 18 to 22 inches in diameter and approx. 42 inches long creating in-running nip point(s) were not adequately guarded on both sides of the PS1 Extruder machine to prevent contact during cleaning and/or other operations.   2/ The PS1 Extruder "die" used to dispense heated plastic in excess of 350 degrees F into the rollers was not adequately guarded to prevent contact when employee(s) are working in that area.    Observed or/or about 01/03/2014 to 01/06/2014.
