29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) At the facility, where the employer did not implement a hearing conservation program for employees exposed to sound levels in excess of 85 dBA.  On April 10, 2014, an employee was exposed to 70.83% of noise dose which is equivalent to approximately 87.5 dBA.  Sampling was conducted for 418 minutes.  Zero exposure was assumed for the remaining 62 minutes.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section:    a) In the Maintenance Shop, where the vat and 55-gallon drum device that is used to dispense 142 Solvent was not labeled.  142 Solvent is a combustible liquid and contains petroleum distillates.     b) In Building 9, where the white plastic 55-gallon drum that contained wire lubricant was not labeled.  The lubricant contains naphthenic distillates.
