29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system or Personal Fall Arrest Systems (PFAS): (a) MG Roofing, 1720 60th Avenue, Greeley, CO: On or about January 6, 2014, employees shoveling snow off the carport roof in preparation to remove and install new shingles were not using Personal Fall Arrest Systems (PFAS). This condition exposed employees to a fall hazard of approximately 9 feet. Abatement Note: Abatement certification is required for this item (See enclosed Abatement-Certification Letter).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surfaces, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed: (a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surfaces, the ladders side rails did not extend at least 3 feet above the upper landing surface being accessed: (a) MG Roofing, 1720 60th Avenue, Greeley, CO: On or about January 6th, 2014, the portable extension ladder used by employees to access the carport roof was set up with the side rails extending approximately two feet above the roof line. Condition exposed employees to a fall hazard of approximately 9 feet while transitioning on and off the snow covered carport roof. Abatement Note: Abatement certification is not required for this item.
