On or about January 03, 2014, at the above addressed job site, an employee was exposed to the hazard of falling approximately 9 feet 7 inches to the ground below, while using a chipping hammer on a second floor balcony without being protected from falling. c. On or about January 03, 2014, at the above addressed job site, employees were exposed to the hazard of falling approximately 28 feet to the ground below, while placing plywood on the roof of three story building without being protected from falling. d. On or about January 06, 2014, at the above addressed job site, employees were exposed to the hazard of falling approximately 28 feet to the ground below, while placing plywood on the roof of three story building without being protected from falling. REPEAT: Habana Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 911661, citation number 1, item number 1 and was affirmed as a final order on 25 Sept 2013, with respect to a workplace located at 81101 Overseas Highway, Islamorada, FL, 33036.
