29 CFR 1910.178(l)(1)(i): Operators were not trained in the safe operation of powered industrial trucks:     (a)A & L Framing & Exterior, Inc. @ 3660 So. Beeler Street, Denver, CO 80237:  On and before January 2nd, 2014, the exposing employer did not ensure each employee who was allowed to operate the All-Terrain Forklift (Genie GTH 644) were trained in the safe operation of Powered Industrial Trucks as required under the standard prior to use. An employee was allowed to operate the rough terrain forklift without completing the training and evaluation specified in paragraph (l). This condition exposed the employees to the hazards of improper and unsafe forklift operations resulting in serious employee injury.    Abatement Note: 29 CFR 1926.602(d): Powered industrial truck operator training. The requirements applicable to construction work under this paragraph are identical to those set forth at 29 CFR 1910.178(l) of this chapter.  Abatement Note:  Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

29 CFR 1926.502(d)(15): Anchorages used for attachment for personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee:   (a)A&L Framing & Exteriors, Inc. @ 1317 14th Street, Denver, CO 80204:  On and before January 2nd, 2014, the exposing employer did not ensure that an independent anchorage capable of supporting at least 5,000lbs was used by an employee engaged in framing activities on a multi-family residence under construction. An employee utilized a rope grab system looped around a roof truss and tied back into itself as a means of anchorage. In the event of a fall, the rope grab system tied back into itself could fail under the dynamic loads imposed due to the fall. The condition exposed an employee to a fall hazard of approximately 10 feet.   Note: 29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed under the supervision of a qualified person, as part of a complete personal fall arrest system (PFAS).

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one stair rail system along each unprotected side or edge:      (a)A & L Framing & Exterior, Inc. @ 3660 So. Beeler Street, Denver, CO 80237:On and before January 2nd, 2014, the exposing employer did not ensure that each stairway(s) used by employees to gain access to the 2nd & 3rd floor of the 3-story residential building under construction, was provided with one stair rail system along each unprotected side or edge.  Every stairway used for access to upper levels in the building was not equipped with stair rail systems along each unprotected side or edge. This condition exposed employees to fall hazards of approximately 10 feet when ascending and descending the stairways.
