29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. (a)United Plumbing Services, Inc. @ 3660 So. Beeler Street, Denver, CO 80237: On or before January 2nd, 2014, the exposing employer did not ensure that each employee conducting work in a 3-story residential building under construction, were provided head protection in the form of protective helmets. This condition exposed employees to a possible danger of head injury from impact, or from falling or flying objects from elevation, or from electrical shock and burns.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one stair rail system along each unprotected side or edge: (a) United Plumbing Services, Inc.@ 3660 So. Beeler Street, Denver, CO 80237. On or before January 2, 2014, the exposing employer did not ensure that each stairway(s) used by employee to gain access to the 2nd & 3rd floor of the 3-story residential building under construction was provided with one stair rail system along each unprotected side or edge. All fights of stairways in the building were not equipped with stair rail system along each unprotected side or edge. This condition exposed the employees to the hazard of falling up to 10 feet when ascending and descending the stairways.
