29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a.  On or about January 06, 2014, at the above job site, an employee was exposed to the hazard of falling, while using an extension ladder that was not secured to prevent displacement and did not extend at least three feet above a landing surface to gain access to a trench that was approximately nine (9) feet deep.    b. On or about January 09, 2014, at the above job site, an employee was exposed to the hazard of falling, while using an extension ladder that was not secured to prevent displacement and did not extend at least three feet above a landing surface to gain access to a trench that was approximately seven (7) feet deep.

On or about January 06, 2014, at the above job site, an employee was exposed to the hazard of falling, while using an extension ladder which was placed on a surface with standing water and soft soil, inside a trench that was approximately nine (9) feet deep.    b. On or about January 09, 2014, at the above job site, an employee was exposed to the hazard of falling, while using an extension ladder which was placed on unstable and unleveled surface soil, inside a trench that was approximately seven (7) feet deep.
