29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system. The employer does not ensure that employees engaged in roofing activities on low-sloped roofs are protected from falling while working at heights greater than six feet. This violation was observed on or about January 07, 2014, when employees were exposed to a 12 foot fall hazards while installing TPO on the roof of a new building without any means of fall protection or monitoring system. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all workers are provided fall protection when working above 6 feet or more above lower levels.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. The employer does not ensure that the ladder side rails used by employee is extended 3 feet above the upper landing surface. This violation was observed at the construction site on or about January 07, 2014, where employees were exposed to a 12 foot fall hazard when using a ladder without the ladder side rails extended 3 feet above the upper landing. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders side rails used by employees are extended 3 feet above the upper landing surface.
