29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a. On or about January 06, 2014, at the above addressed job site, employees were exposed to the hazard of falling approximately 28 feet to the ground below while unloading materials from a man-lift onto the roof of a home that did not have a proper fall protection system in place.

On or about January 06, 2014, at the above addressed job site, employees were exposed to the hazard of falling approximately 9 feet to the floor below while using a staircase to gain access to the second floor which had unprotected sides.
