29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions: Jobsite: The employees, who were working on and around energized electrical equipment were not wearing the appropriate personal protective equipment (PPE) and were exposed to the electrically related hazards, such as (but not limited to) severe electrical shocks, arc flash/blast burns and electrocution. On or about January 8, 2014, two (2) employees were working on energized 480-volt electric distribution panel without wearing appropriate PPE, and one of employees sustained very severe electrical burns from the elecric arc flash/arc blast.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. Jobsite: Employees were permitted to work on and around energized electrical power circuits and were not adequately and appropriately protected from electrically related hazards, such as (but not limited to) severe electric shocks, are flash/blast burns and/or electrocution by utilizing appropriate means, such as (but not limited to) wearing proper types of personal protective equipment (PPE), de-energizing and grounding the circuits, and utilizing proper types of tools. On or about January 8, 2014, two (2) employees were permitted to work on energized 480-volt electric distribution panel, and the employees were neither wearing proper types of PPE nor utilizing proper types of tools while performing the tasks. One of employees sustained very severe electric burns to both arms where the second employee sustained minor burns to eyes from the electric arc flashes/blasts.
