29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:   a. On or about January 06, 2014, at the above addressed job site, employees were exposed to the hazard of falling approximately 28 feet to the ground below while unloading materials from a man-lift onto the roof of a home that did not have a proper fall protection system in place.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  a. On or about January 06, 2014, at the above addressed job site, employees were exposed to the hazard of falling approximately 9 feet to the floor below while using a staircase to gain access to the second floor which had unprotected sides.
