29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:      (a)    Antonio Paredes Alvarez @ 5200 W. 43rd Ave., Denver, CO.80212. On or before December 13, 2013, the exposing employer did not ensure that each employee working on the ground picking up debris from the roofers working above was provided hard hats. This condition exposed employees to a possible danger of head injury from impact, or from falling or flying objects from the roof.

29 CFR 1926.501(b)(11): Each employee on a steep-slope roofs with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, or personal fall arrest systems (PFAS).      (a)    Antonio Paredes Alvarez @ 5200 W. 43rd Ave., Denver, CO.80212. On or before December 13, 2013, the exposing employer did not ensure that each employee was provided adequate fall protection systems while working on the steep-sloped roof(s) (6 in 12, 10 in 12 pitch) performing roofing activities on the Berkeley Baptist Church. This condition exposed employees to the hazard of falling up to 21 feet eave to ground in the following instances:    (1)    On December 13, 2013, two employees working on the roof were using a single anchorage instead of each employee having a separate and individual anchorage capable of supporting at least 5,000 lbs. per each employee attached. This condition exposed employees to the hazard of falling up to 21 feet eave to ground.       (2)    On December 13, 2013, one employee working on the roof was inadequately tied off. He used the "D" ring positioned on his chest instead of on his back in the center of the shoulder blades for proper support and arrest. This condition exposed the employee to the hazard of falling up to 21 feet eave to ground.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured to provide safe access.      (a)    Antonio Paredes Alvarez @ 5200 W. 43rd Ave., Denver, CO.80212. On or before December 13, 2013, the exposing employer did not ensure that the extension ladder used by employees to gain access to the roof was extended at least 3 feet above the roof and secured at its top to prevent displacement.   This condition exposed the employee to the hazard of falling up to 19 feet eave to ground.
