29 CFR 1926.451(g)(3): In addition to meeting the requirements of 1926.502(d), personal fall arrest systems used on scaffolds shall be attached by lanyard to a vertical lifeline, horizontal lifeline, or scaffold structural member. Vertical lifelines shall not be used when overhead components, such as overhead protection or additional platform levels, are part of a single-point or two-point adjustable suspension scaffold.  a)  Burro located on Ramp #2;  On or about January 8, 2014 and at times prior thereto, the employer exposed employees to fall hazards of approximately 93.5 feet in that employees were permitted tie-off to the guardrail system of a self-contained powered scaffolding (referred to as the Burro) while wrecking formwork from the side of the ramp.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:  a)  Miller wagon located on Ramp #2;  On or about January 8, 2014 and at times prior thereto, the employer exposed employees to fall hazards of approximately 93.5 feet in that employees were required to tie-off to a horizontal lifeline that was not designed, installed, or used under the supervision of a qualified person while working in the basket of the Miller wagon wrecking formwork from the side of the bridge.

29 CFR 1926.502(d)(23): Personal fall arrest systems were attached to guardrail systems or to hoists:   a)  Miller wagon located on Ramp #2;  On or about January 8, 2014 and at times prior thereto, the employer exposed employees to fall hazards of approximately 93.5 feet in that employees were required to tie-off to a horizontal lifeline that was installed across the guardrails of the Miller wagon basket that was positioned over the side of a ramp while employees were wrecking formwork.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:   a)  Ramp #2;  On or about January 8, 2014 and at times prior thereto, the employer exposed employees to fall hazards in that frayed lanyards on the employees personal fall arrest system were not removed from service while working on a ramp that was approximately 93.5 feet high.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  East-side (Miller wagon-side) of Ramp #2;  On or about January 8, 2014 and at times prior thereto, the employer exposed employees to fall hazards of approximately 93.5 feet when employees leaned over the 29 inch high rebar barrier wall at the edge of the ramp to retrieve formwork materials that had been wrecked from the side of the ramp without wearing any type of fall protection.    b)  West-side (Burro-side) of Ramp #2;  On or about January 8, 2014 and at times prior thereto, the employer exposed employees to fall hazards of approximately 93.5 feet when employees leaned over the 29 inch high rebar barrier wall at the edge of the ramp to retrieve formwork materials that had been wrecked from the side of the ramp without wearing any type of fall protection.
