29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: (a) At West Agro, Inc. on or about January 8, 2014 the aisle at the west end of A Cell was obstructed by steps and stored material sticking out into the aisle narrowing the aisle to 18 inches. The edge of the platform and steps was not marked with a yellow line to make the steps vissible creating a tripping hazard.

A hook on the Dayton hoist located near the entrance to the shop had a hook with a damaged keeper guard and inspection record for the hoist, chain and hook was not available.
