Air contaminant monitoring was not available for employee(s) exposed to high levels of carbon monoxide from a gasoline powered pump and propane forklift exhaust in the DEF production/warehouse area. On December 7, 2013, an employee suffered severe carbon monoxide poisoning from the unmonitored atmosphere in his work area.

On December 7, 2014, employee(s) working in the DEF production/warehouse area were exposed to carbon monoxide levels that exceeded the permissible exposure limit listed in Table Z-1 of 29 CFR 1910.1000. An employee suffered carbon monoxide poisoning and had a post exposure carboxyhemoglobin blood level of 28.5%.
