29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the skylight:    Employees re-roofing the two story house at 4030 N. Tara Circle, Wichita Kansas were exposed to fall hazards approximately 8 feet into the house through the skylight.  The skylight on the 12 in 12 pitch roof was not protected to keep employees from falling through.

Tara Circle, Wichita Kansas were exposed to fall hazards approximately 9 feet 3 inches on the 12 in 12 pitch roof to the ground below.  The employees did not use any fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Employees re-roofing the two story house at 4030 N. Tara Circle, Wichita Kansas were exposed to fall hazards of up to approximately 14 feet on the 12 in 12 pitch roof to the ground below.  None of the three ladders used to access the roof extended above the eaves at least three feet.

Tara Circle, Wichita Kansas were exposed to fall hazards of approximately 15 feet to the ground below in that employees were not trained in the correct way to setup ladders.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Employees re-roofing the two story house at 4030 N. Tara Circle, Wichita Kansas were exposed to fall hazards of 9 feet 3 inches to the ground below in that employees were not trained to recognize when they could no longer use fall protection when installing ridge caps.  Vega Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1925.503(a)(1), which was contained in OSHA inspection number 312802069 , citation number 01, item number 002 and was affirmed as a final order on 08/27/2009 , with respect to a workplace located at 801 W. Maple St., Wichita, Kansas.
