29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:  (a) Warehouse Building at 209 Butler Avenue, York, Pennsylvania - Two employees worked installing flashing on the low-sloped metal roof on the portion of the building that ran perpendicular to Butler Avenue.  The roof had an eave height of 11 feet above the ground and a width of 40 feet.  The employees did not use a safety monitoring system or any other fall protection system to prevent them from falling to the ground below, on or about January 9, 2014.  No abatement certification or documentation required.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a) Warehouse Building at 209 Butler Avenue, York, Pennsylvania - The employer did not have a competent person involved in the job site in that neither of the two employees who worked installing flashing on the low-sloped metal roof on the portion of the building that ran perpendicular to Butler Avenue had training on or knowledge of the requirements for fall protection for roofing activities on low-sloped roofs with unprotected sides or edges 6 feet or more above lower levels, on or about January 9, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Warehouse Building at 209 Butler Avenue, York, Pennsylvania - The employer did not provide fall protection training to employees who worked installing flashing on the low-sloped metal roof on the portion of the building that ran perpendicular to Butler Avenue to ensure that these employees had knowledge of the requirements for fall protection for roofing activities on low-sloped roofs with unprotected sides or edges 6 feet or more above lower levels, on or about January 9, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
