29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: The employees operating the cheese flinger machine on the flatbread line were exposed to entanglement hazards in that, the cover did not prevent employees from being able to reach the point of operation while the machine was running.
