29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:     (a) No noise monitoring program had been implemented by the employer. OSHA measured ambient sound levels at approximately 76 decibels. The loudest sound levels were measured at approximately 107 decibels when an employee was grinding. Impact noises were consistently present and measured to be over 90 decibels.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by:  (a) South wall, Production area; Employee(s) who use the Delta 20 vertical Band Saw, were exposed to laceration and amputation hazards when the exposed unused portion of the blade was not guarded while they cut excess metal off cast parts.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Production Area: A written hazard communication program was not developed or implemented when hazardous chemicals were used by employees in the workplace. Hazardous chemicals used at this facility include Aluminum which is potentially explosive in dust form. Aluminum dust is created during grinding operations. Aluminum 2217 contains amounts of silicon, iron, copper, manganese, magnesium, chromium, nickle, zinc, titanium, cadmium, gallium, lead and antimony.
