29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition.  a) Throughout the Resaw building: On January 9, 2014 and at times prior thereto, more than 5% of the total square footage of the floor and horizontal surfaces were covered in wood dust measuring 7/32 to 12/32 of an inch on horizontal surfaces exposing employees to fire and explosion hazards associated with combustible dust. 29 CFR 1910.219(c)(4)(i): Projecting shaft ends did not present a smooth edge and end and projected more than one-half the diameter of the shaft and were not guarded by nonrotating caps or safety sleeves:    a)    In the saw mill area: On or about June 24, 2013, and at times prior thereto, shafts ends at the out-feed conveyor were unguarded.     b)    In the saw mill area: On or about June 24, 2013, and at times prior thereto, shaft ends on the Pacific Trails chain saw (Bunk saw) was unguarded and exceeded one half the diameter of the shaft. Specialty Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(c)(4)(i) which was contained in OSHA inspection number 29646, citation number 01, item number 002 and was affirmed as a final order on 08/10/2011, with respect to a workplace located at 4323 Pioneer Rd. in Homedale, ID. 83628.
