29 CFR 1904.7(b)(4): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent.      At the Facility - The employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2013:       a)    On 7/30/2013, an employee received a chemical burn.  He lost 4 days of work. The OSHA 300 log did not indicate he had lost any time nor was column H checked.       b)    On 10/4/2013, and employee felt something in his back "pop".  This resulted in 6 days of restricted work.  The OSHA 300 log did not indicate he had received any restricted time nor was column I checked.       c)    On 12/5/2013, and employee fell form a chair and hit her head.  This resulted in 11 days away form work.  The l11days was not recorded on the OSHA 300 log nor was the incident included in the totals for column H.
