29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a) PalPac Industries, Inc - Ottawa, Ohio: On or about January 28, 2014, employees were exposed to fall hazards of approximately 52 inches when accessing the platform located around Press 4.  The platform was not equipped with a standard guardrail that included a mid-rail.    b) PalPac Industries, Inc. - Ottawa, Ohio: On or about January 28, 2014, employees were exposed to fall hazards of approximately 52 inches when accessing the platform adjacent to Press 4.  A standard guardrail, or equivalent fall protection, was not provided to prevent falls in the mold area of the press to the ground below.    c) PalPac Industries, Inc. - Ottawa, Ohio: On or about January 27, 2014, employees were exposed to fall hazards of approximately 58 inches when accessing the platform adjacent to Press 5.  A standard guardrail, or equivalent fall protection, was not provided to prevent falls in the mold area of the press to the ground below.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) PalPac Industries, Inc. - Ottawa, Ohio: On or about January 27, 2014, the employer did not develop and implement an energy control program to protect employees from hazardous energy sources while setting up molds and maintaining presses.  Employees were exposed to amputation hazards while servicing equipment such as, but not limited to, Presses 1, 3, 4, 5, and 6.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) PalPac Industries, Inc. - Ottawa, Ohio: On or about January 22, 2014, employees were not protected from crushing injuries in that the pinch point created by the two platens of Press 1 were not adequately guarded to prevent employees from accessing the hazard area at a time the press could operate.    b) PalPac Industries, Inc. - Ottawa, Ohio: On or about January 20, 2014, employees were not protected from crushing injuries in that the pinch point created by the two platens of Press 3 were not adequately guarded to prevent employees from accessing the hazard area at a time the press could operate.    c) PalPac Industries, Inc. - Ottawa, Ohio: On or about January 29, 2014, employees were not protected from crushing injuries in that the pinch point created by the two platens of Press 4 were not adequately guarded to prevent employees from accessing the hazard area at a time the press could operate.
