Employees were exposed to mechanical and gravitational hazardous energy sources while performing set-up operations. b) Matsu Ohio, Inc. - Edgerton, Ohio: On or about January 8, 2014, lockout procedures for the Niagara 400-25 (s/n 52667) straight side mechanical power press did not identify specific steps to control hazardous energy sources, contain instruction for placement of energy control devices, or how to determine the effectiveness of energy control procedures. Employees were exposed to mechanical and gravitational hazardous energy sources while accessing the die space to set date stamps. c) Matsu Ohio, Inc. - Edgerton, Ohio: On or about January 9, 2014, lockout procedures Minster #400-36 (s/n E2-400-27391) straight side mechanical power press did not identify specific steps to control hazardous energy sources, contain instruction for placement of energy control devices, or how to determine the effectiveness of energy control procedures. The employer did not assure that mechanical and gravitational hazardous energy sources were isolated/controlled. b) Matsu Ohio, Inc. - Edgerton, Ohio: On or about January 8, 2014, employees were exposed to crushing and/or amputation hazards when accessing the hazard area of the Niagara 400-25 (s/n 52667) press to set a die date stamp. The employer did not assure that mechanical and gravitational hazardous energy sources were isolated/controlled before employees serviced the press. c) Matsu Ohio, Inc. - Edgerton, Ohio: On or about January 9, 2014, employees unjamming parts in the hazard area of the Minster #400-36 press (s/n E2-400-27391) were exposed to crushing and/or amputation hazards. Fixing of selection was not capable of supervision by the employer: a) Matsu Ohio, Inc. - Edgerton, Ohio: On or about January 9, 2014, the employer did not assure that the operating mode on the Niagara 400-25 (s/n 52667) mechanical power press was capable of being set in a fixed position. Employees were exposed to an amputation hazard in that the operating mode could be changed by a selector switch that was not capable of being secured in a fixed position and supervised by the employer. b) Matsu Ohio, Inc. - Edgerton, Ohio: On or about January 9, 2014, the employer did not assure that the operating mode on the USI Clearing (SE4-1000-108-96) mechanical power press was capable of being set in a fixed position. Employees were exposed to an amputation hazard in that the operating mode could be changed by a selector switch that was not capable of being secured in a fixed position and supervised by the employer.
