29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer does not ensure that lockout or tagout devices are affixed to each energy isolating device. This violation occurred on or about January 9, 2014, at the facility where maintenance employees did not affix a lockout device to the ball valve for pneumatic energy when shutting down Strander #1 to perform service and maintenance tasks. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that lockout or tagout devices are affixed to each energy isolating device.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: The employer does not ensure that each authorized employee affixes a personal lockout or tagout device before working on the machine or equipment. This violation occurred on or about January 9, 2014, at the facility where each maintenance employee performing service and maintenance tasks on Strander #1 did not apply a personal lockout or tagout device. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that each authorized employee affixes a personal lockout or tagout device before working on the machine or equipment.
