29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about 1/10/2014, in the Metal Finishing Department, the eye wash station that was provided did not have clear water when turned on, exposing employees to hazards of injuries to the eyes while using corrosive chemicals, including, but not limited to, sodium hydroxide.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  In the Short Run Department, the Grob cylindrical thread roller machine was not guarded, exposing employees to hazards created by the rotating cylinders.    (b)  In the Short Run Department, the foot control for the Grob cylindrical thread roller machine had been attached to the top of the machine, exposing employees to hazards created by the rotating cylinders.    (c)  In the Short Run Department, the #2 Cincinnati centerless grinder was not guarded, exposing employees to hazards created by the rotating grinding wheel and the rubber wheel.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:     (a)  On or about 1/10/2014, in the Short Run Department on the thread roller machine, employees were not provided hand tools for holding bolts between the cylinders for threading, exposing employees to hazards of being caught in the rotating cylinders.     (b)  On or about 1/10/2014, in the Short Run Department on the Cincinnati centerless grinder, employees were not provided hand tools for holding bolts between the grinding wheel and rubber wheel, exposing employees to hazards of being caught in the rotating cylinders.
