On or about January 9, 2014, and at times prior thereto, employee(s) were exposed to a struck-by hazard from bundles of rebar weighing up to 6000 pounds being transported on a Shaw Box 3-ton gantry crane #2, that had the the hook clip missing, while working in the vicinity of the crane.

29 CFR 1910.179(j)(2)(iv): Daily visual inspection of the hoist chains, and monthly inspection with a certification record was not accomplished which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected: a) Fabrication shop. On or about January 9th, 2014 and at times prior thereto, monthly inspections of ropes in the crane assembly, used to move loads of rebar weighing 4000-6000 pounds throughout the shop where employees are working, were not documented on the 3-ton Shaw Box gantry crane #1. b) Fabrication Shop. On or about January 9, 2014, and at times prior thereto, employee(s) were exposed to a struck-by hazard from bundles of rebar weighing up to 6000 pounds being transported on a Shaw Box 3-ton gantry crane #2, that had the the hook clip missing, while working in the vicinity of the crane. c) Fabrication shop.
