29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means:  On or about January 3, 2014, at the worksite located at 831 West Center Street in Fayetteville, Arkansas, Level 2 of Pad G, employees were not protected against electrical shock in that they were allowed to use a bull-float equipped with an aluminum handle to perform concrete finishing tasks in proximity to energized overhead power lines.  This condition exposed employees to electrocution from indirect contact with a high voltage power line.

29 CFR 1926.416(a)(3): Employer did not post or maintain proper warning signs at locations where the performance of the work could bring a person, tool, or machine into physical or electrical contact with an energized electrical power circuit:  On or about January 3, 2014, at the worksite located at 831 West Center Street in Fayetteville, Arkansas, Level 2 of Pad G, employees used a bull-float equipped with an aluminum handle to perform concrete finishing tasks in proximity to energized overhead power lines.  Warning signs were not posted to alert employees contact could be made with the energized power lines.  This condition exposed employees to electrocution from indirect contact with a high voltage power line.

Bull float handles used where they might contact energized electrical conductors, shall be constructed of nonconductive material or insulated with a nonconductive sheath whose electrical and mechanical characteristics provide the equivalent protection of a handle constructed of nonconductive material:  On or about January 3, 2014, at the worksite located at 831 West Center Street in Fayetteville, Arkansas, Level 2 of Pad G, employees used a bull-float equipped with an aluminum handle to perform concrete finishing tasks in proximity to energized overhead power lines.  This condition exposed employees to electrocution from indirect contact with a high voltage power line.
