29 CFR 1910.147(c)(5)(i):  Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:      (a)  Employees were not provided with locks or tags to attach to the energy isolating device, prior to changing the blades on the Behringer Horizontal Band Saw (Machine #SW-503), the Behringer Horizontal Band Saw (Machine #SW-502), the Haberle Chop Saw (Machine #18458).       (b)  Employees were not provided with locks or tags to attach to the energy isolating devices prior to changing the shear blade, shear tube, die and guides on the Schumag Coil Line.    (c)  Employees were not provided with locks or tags to attach to the energy isolating device prior to changing, or putting back on, the vertical band saw blade on the McKay Line.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. (b)  Employee training on the energy control program was inadequate, in that it did not include training employees on when and how to lock out the Behringer Horizontal Band Saw (Machine #SW-503), the Behringer Horizontal Band Saw (Machine #SW-502) and the Haberle Chop Saw (Machine #18458), prior to changing the blades. 29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:     (a)  The pre-straightener rollers and transport rollers on the Schumag Coil Line were not guarded, thereby exposing employees to in-going nip points.      (b)  The shear on the Schumag Coil Line was not guarded, thereby exposing employees to the blade.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
