29 CFR 1926.501(b)(12): Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems, unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   On or about November 20, 2013, employees were exposed to a fall hazard, greater than 6 feet, to the dirt ground below while moving/landing a pre-cast concrete member (Spandrel) to the perimeter of the garage under construction.  The employees grabbed the tag line while standing near the south edge of the 2nd level of the garage and escorted the precast member/load as it flew overhead, via a crane, to the north side of the garage, thus being exposed to the unguarded perimeter on both the south and north sides of the 2nd floor garage.
