29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:     a)      Brookside Contracting, Inc. on or about January 10, 2014, at the address of 1094 S. Main St. Bowling Green OH: The employer failed to ensure employees working from an aerial lift were adequately protected from falls.  The employees working from the lift were not clipped off to the basket while the platform was elevated. The employees were exposed to falls of up to 14 ft. to the pavement below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  Brookside Contracting, Inc. on or about January 10, 2014, at the address of 1094 S. Main St. Bowling Green OH: The employer did not ensure employees were protected from falls while performing wood framing work on a commercial type structure.  During this work, employees were exposed to falls up to 14 ft. to the pavement below.

Main St. Bowling Green OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various wood framing operations from elevated surfaces.
