29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about January 10, 2014, one employee was observed using a 20-foot Extension Ladder positioned on the side of the apartment complex under construction, which did not have the ladder side rails extend at least 3 feet (.9 m) above the roof surface. The employee was exposed to a fall hazard of greater than six feet.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about January 10, 2014, to the right side of the house, employees were using an orange 100-foot electrical extension cord (to power a Makita 7 ¼ inches Circular Saw, Model #5007F) with the strain relief broken on the female side. Employees were exposed to electrical shock hazards.
