An electrical outlet was missing a faceplate.  c)  On January 10, 2014, the electrical outlets in the Wausau location were not visually inspected before use.  Two electrical outlets were not properly installed on the wall.  d) On January 10, 2014, an electrical conduit was not insulated properly, was broken, and live.
