29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Mojo Pad- 1451 Amity Ridge Road, Amwell Township, PA: On or about January 10, 2014, employees were exposed to struck-by hazards from residual gas which had not been properly vented by the company while changing sand straws to the Gas Production Unit. The employer did not develop written procedures specific to this task.
