29 CFR 1910.303(d): Parts of electric equipment that in ordinary operation produce arcs, sparks, flames, or molten metal were not enclosed or separated and isolated from all combustible material. The employer did not ensure that electrical parts were enclosed. This violation occurred on or before December 27, 2013, at a job site located at 3114 S. Haskell, Dallas, Texas 75223; where employees using the palletizer machine was exposed to arc flash.
